How do I get to the office/hospital?     

Please note: Unfortunately, there is no elevator at either location, and each office is on the second floor. Please let us know if this will present a problem in getting to the office.

Hwy 5 South to 805. 

Exit at Balboa, East. 

Go East about one mile. 

Pass over Hwy 163. 

Left on Kearny Villa Rd. 

U-turn at first stop light. 

Right into Landmark Centre. 

Go to back building.

Hwy 5 South to Lomas Santa Fe  Drive.

Go Left (East) under freeway one block. 

Turn Right into Lomas Santa Fe Plaza (entrance near Ross store).

Yes, unless your insurance requires us to bill them directly. Payment of co-pay is requested at the time of the visit.

How much is my co-pay?

How can I get a prescription refilled?

Please ask your pharmacy to fax the request to Dr. Gardner, or ask Dr. Gardner during your appointment time.

How can I find out results to my tests?

Please ask Dr. Gardner during your appointment time.
